What do I bring to my appointment?
Arrive at your appointment with your Patient Registration Form. You can download it below , complete the form, print it, and bring it with you.
You may review our practice’s privacy policy here. You do not need to print or bring this document to your appointment.
If you have been referred for physical therapy by your doctor, please bring your prescription. Please note that a referral is not always needed. We can help determine whether or not you require a referral for treatment.
If your insurance is covering the cost of physical therapy, bring your insurance card. If you are covered by Workers’ Compensation, bring your claim number and your case manager’s contact information. If you are covered by auto insurance or an attorney lien, make sure you bring this information. Cash payment and treatment packages are also available.
